In Q-SYMBIO, a single mid-sized trial of 420 patients with chronic heart failure, major cardiac events over two years were fewer on top of standard treatment (15% vs 26%). In healthy adults it reliably raises blood levels, but a 2025 meta-analysis found effects on endurance and strength small, inconsistent and of low certainty. Some meta-analyses report slightly less fatigue.

“CoQ10 prevents steroid damage to the heart.”
There is no evidence that it prevents steroid-related heart thickening or arrhythmia. What lowers (but does not remove) the heart risk is blood pressure and lipid control, cardio and screening (ECG, echo).
