Topical antibacterial for acne: an oxidizing agent that kills acne bacteria and loosens blocked pores. One of the main first-line acne treatments.

Purpose

Used for 'steroid acne' on the back, chest and face, one of the most common visible side effects of anabolic steroids. Medical use: mild to moderate acne, alone or with a topical retinoid or antibiotic; strongly recommended in the AAD 2024 acne guideline.

What people seek and what studies show

Used to clear acne driven by steroids. In 6 placebo-controlled trials (4,110 people) doctors rated improvement about 1.8 times as often with it, though the certainty of the evidence is very low; it performed about as well as adapalene. Unlike topical antibiotics, it does not breed resistant bacteria. It has not been studied in steroid users, whose oil glands stay under androgen drive.

Fact

It does not induce bacterial resistance and also kills antibiotic-resistant acne bacteria, which is why it is paired with antibiotics to limit resistance to them.

Isotretinoin already dries the skin and makes it fragile. Adapalene, a second retinoid, and benzoyl peroxide irritate and peel it too, and the isotretinoin product information says to avoid such treatments alongside it because irritation increases: redness, burning, cracks and peeling.
