Azelaic acid
Skinoren, Finacea, Azelex, Azelik, Azix-Derm, AzA
Forms: Cream for acne; gel and foam forms are mainly used for rosacea.
Topical acid, also present in whole grains, with mild antibacterial and pore-unclogging action; it may also fade dark marks. A second-line acne option.
Legal status: Prescription in the US; elsewhere a prescription or pharmacy product; varies by country. Not on the WADA Prohibited List.

Purpose
Used for steroid acne and the dark marks it leaves. Medical use: mild to moderate inflammatory acne and rosacea; the AAD 2024 acne guideline gives it a conditional recommendation.
What people seek and what studies show
Used to calm inflamed spots and fade dark marks. A Cochrane review found it probably less effective than benzoyl peroxide (one trial, 351 people) and about as effective as tretinoin; in another trial benzoyl peroxide with clindamycin cut inflamed spots more (79% vs 65%). A small uncontrolled pilot study suggested it fades post-acne dark marks. No data in steroid users.

For women
About 4% of what is applied is absorbed. There are no good studies in pregnancy, so the label advises use then only if clearly needed; caution while breastfeeding.

In a direct comparison with benzoyl peroxide it was probably less effective; its extra value is a modest effect on dark marks, shown so far only in small studies.
