Adapalene

Forms: Gel, cream or lotion in two strengths; the weaker gel is sold over the counter in the US. Fixed combination with benzoyl peroxide (Epiduo, Effezel).
Topical retinoid (a vitamin-A-like drug) that normalizes how skin cells renew inside pores and calms inflammation; a first-line acne treatment.
Legal status: The weaker gel is over the counter in the US; elsewhere it may need a prescription; varies by country. Not on the WADA Prohibited List.

Purpose
Used for steroid-driven acne, which affects about half of anabolic steroid users. Medical use: acne vulgaris from age 12; topical retinoids are strongly recommended in the AAD 2024 guideline.
What people seek and what studies show
Used to clear blackheads and inflamed spots. In a trial of 653 people with mild to moderate acne, the weaker gel cleared or almost cleared skin in 16% vs 9% on a plain gel and cut inflamed spots by about half (vs 41%). With benzoyl peroxide in moderate to severe acne, 34% vs 11% reached that goal. Not studied in steroid users.

For women
A retinoid: high oral doses caused birth defects in animals, though very little reaches the blood from skin. Labels say to ask a doctor if pregnant or breastfeeding and to stop if pregnancy occurs.
Myth & fact
“If acne gets worse in the first weeks, adapalene is making it worse.”
An early flare and irritation are expected and usually settle; results can take up to three months. Severe irritation or no improvement is a reason to see a doctor.
Combinations
Isotretinoin already dries the skin and makes it fragile. Adapalene, a second retinoid, and benzoyl peroxide irritate and peel it too, and the isotretinoin product information says to avoid such treatments alongside it because irritation increases: redness, burning, cracks and peeling.
