Ketoconazole shampoo

Nizoral, Nizoral A-D

Forms: Shampoos of different strengths, some combined with zinc pyrithione; creams for the skin. Ketoconazole tablets are a different matter: in Europe they were suspended for fungal infections in 2013 over liver injury.

Health support (prescription)Over the counter (varies by country)Evidence: approved medicine

Antifungal shampoo that reduces Malassezia yeast on the scalp and skin, used for dandruff and seborrheic dermatitis. It is not detected in the blood after shampooing.

What it gives

What people use it for and what the sources report. Strength is a general category from the literature: mild, clear or strong.

Purpose

Used by steroid users as a mild add-on against thinning hair, and by dermatologists for itchy yeast folliculitis on the sweaty back and chest that looks like steroid acne. Medical use: dandruff, seborrheic dermatitis, pityriasis versicolor.

What people seek and what studies show

Hair: a review of five small human studies (318 people) found thicker hair shafts and more growing hairs; in one study density gains were close to minoxidil's. Randomized trials are lacking. Skin: in 110 young people with yeast folliculitis, mostly after antibiotic-treated acne, ketoconazole shampoo was the usual treatment and most improved. Not studied in steroid users.

How to lower the risks

For 1 of 1 possible side effect there are specific ways to lower the chance: everyday measures, supplements and options a doctor may consider. No measure removes a risk completely.

Low-level side effects

Hair loss
Ketoconazole shampoo, minoxidil, a dermatologist early if you are prone to it.
Supplement or OTC product
How each option helps (1)
  • Minoxidil (topical) — Acts on the hair follicle, not on hormones, so it may help whatever steroid drives the loss: slows thinning and regrows some hair. Scalp itch and early shedding are common; gains fade once stopped.

Check-ups and blood tests

Which tests and check-ups to do and when. This is a guide — agree the exact schedule with a sports physician.

The sources list no specific tests for this substance on its own. Together with other substances, the stack check shows the tests for the whole combination.

For women

It was not detected in the blood even after months of shampooing; for pregnancy and breastfeeding labels still advise caution because studies are lacking.

Myth & fact

Myth

“Ketoconazole shampoo blocks DHT like finasteride.”

Fact

It works on the scalp surface: ketoconazole was not found in the blood even after months of shampooing, and its hair benefit in small studies is modest.

Combinations

No specific combination notes for this substance yet. The stack check still adds up side effects that several substances share.
