Essential phospholipids (Essentiale)

Essentiale Forte, Essentiale Forte N, EPL, Polyenylphosphatidylcholine, PPC, Lipostabil

Forms: Purified soybean polyenylphosphatidylcholine rich in dilinoleoylphosphatidylcholine; ordinary lecithin supplements are less purified and are not the same product.

SupplementsOver the counter (varies by country)Evidence: limited human data

Purified phosphatidylcholine extract from soybean, a building block of cell membranes. Sold as a liver remedy; most clinical studies come from Europe and Asia.

What it gives

What people use it for and what the sources report. Strength is a general category from the literature: mild, clear or strong.

Purpose

Taken by steroid users to try to avoid liver toxicity, and medically as supportive therapy for fatty liver. Evidence is weak: one non-randomised study in steroid users and small trials with unclear or high risk of bias.

What people seek and what studies show

In a non-randomised study of 160 steroid users, the 44 who also took a phospholipid-vitamin product had no rise in liver enzymes, unlike the others; the design cannot prove cause. A two-year placebo-controlled trial in 789 heavy drinkers found no effect on fibrosis progression. In fatty liver with diabetes or obesity, small trials report lower ALT and triglycerides (manufacturer-funded analysis).

Possible side effects

Possible does not mean certain: whether they appear depends on dose, duration, individual response and monitoring. Below — what to watch for and how to notice it early.

level · how often it is reported
Low
Nausea, GI upset, pancreatitis risk
Occasional mild stomach discomfort or loose stools; no serious adverse events in trials.
How to notice: Nausea, fullness, constipation or diarrhea.

How to lower the risks

For 1 of 1 possible side effect there are specific ways to lower the chance: everyday measures, supplements and options a doctor may consider. No measure removes a risk completely.

Low-level side effects

Digestion
Smaller meals, enough fluid and fiber; a doctor for severe abdominal pain.
Measure
Working with a doctor
Supplement or OTC product
How each option helps (2)
  • Working with a doctor — Assesses persistent vomiting or severe belly pain (possible pancreatitis or gallstones) and adjusts prescribed GLP-1 treatment when nausea persists.
  • Pancreatin + dimeticone (Pankreoflat) — May ease heaviness and gas after meals; can cause nausea or diarrhea. Not for pork or cow's milk protein allergy (anaphylaxis reported) or early acute pancreatitis.

Check-ups and blood tests

Which tests and check-ups to do and when. This is a guide — agree the exact schedule with your doctor.

Myth & fact

Myth

“With Essentiale, oral steroids won't hurt the liver.”

Fact

The only study in steroid users was not randomised, and a large two-year trial found no effect on liver fibrosis. 17α-alkylated orals can still cause cholestasis; liver tests show what is happening.
