Milk thistle (silymarin)
Silybin, Silibinin, Legalon, Carsil
Forms: Standardized silymarin extracts and silibinin (silybin), including phytosome forms; teas contain very little silymarin.
Seed extract (silymarin) of a Mediterranean plant, sold as a liver tonic. Well tolerated, though people allergic to ragweed or daisies can react.
Legal status: Sold freely as a herbal supplement (a licensed herbal medicine in some countries); silymarin content and purity vary by brand.
What it gives

Purpose
Taken as 'liver support' while using oral steroids or after alcohol. Lab and animal data look promising, but trials in hepatitis C and fatty liver disease found little or no benefit.
What people seek and what studies show
In a NASH trial (fatty liver with inflammation) it missed its main goal, though more patients had less fibrosis on biopsy, still unconfirmed. A Cochrane review of 18 trials in alcoholic and viral liver disease found no significant effect on death, complications or liver tissue. Small, low-quality trials hint at lower blood sugar in type 2 diabetes. Purified silibinin is used in hospitals for death-cap poisoning.
