Alpha-1 blocker that relaxes the muscle of the prostate and bladder neck, easing a weak stream and frequent urination. It does not shrink the prostate.
Legal status: Prescription medicine in most countries; varies by country. Not on the WADA Prohibited List.
What it gives

Purpose
Urologists prescribe it for urinary symptoms of an enlarged prostate, including in older men using androgens. Medical use: symptoms of benign prostatic enlargement; strongly recommended for moderate to severe symptoms in the EAU guideline.
What people seek and what studies show
Used to ease urinary symptoms. In controlled studies alpha-blockers cut symptom scores by about 30–40% and raised urine flow by about 20–25%, though placebo groups also improved a lot; effects can start within hours to days. They do not reduce prostate size or prevent acute urinary retention; adding a 5α-reductase inhibitor lowers that risk. No data in androgen users.

Myth & fact
“Tamsulosin shrinks an enlarged prostate.”
It relaxes muscle in the prostate and bladder neck so urine flows more easily; the gland stays the same size. Shrinking it is what 5α-reductase inhibitors do.
Combinations
Both widen blood vessels and can lower blood pressure, and the tamsulosin label advises caution with PDE5 inhibitors: dizziness or fainting on standing up is more likely, especially with heat, dehydration or alcohol. Urologists do combine them for urinary and erection problems, under supervision.
