Purpose

A doctor may consider it when readings show high blood pressure, common on AAS. Unlike sartans and ACE inhibitors it does not raise potassium, which matters with spironolactone or electrolyte mixes. Medical use: hypertension, angina.

Used to lower blood pressure. In ALLHAT, with 33,357 hypertensive patients, it prevented heart attacks and coronary deaths as well as a diuretic, though heart failure was more common (10.2% vs 7.7%). In trials it did not meaningfully change potassium, blood sugar or cholesterol. Not studied in steroid users.

For women

Ankle swelling and flushing are about two to three times more common in women. Data in pregnancy are too limited to judge the risk — decide with a doctor.

Myth

“Swollen ankles on amlodipine mean high estrogen and water retention.”

Fact

The swelling is thought to come from widened small arteries raising pressure in the capillaries, not from estrogen. In trials of this drug class it grew with dose and time, reaching about a quarter of users after 6 months.

Tadalafil lowers blood pressure a little, and so do these drugs; the effects add up. With dehydration, heat or alcohol this can mean dizziness or fainting, especially when standing up quickly.
