Tadalafil
Cialis, Adcirca, Tadalis
Forms: Tablets for erectile and prostate symptoms (Cialis) and for pulmonary hypertension (Adcirca).
Long-acting PDE5 inhibitor that improves erections and eases urination with an enlarged prostate. Prescription drug, only under a doctor's supervision; never with nitrates or 'poppers'.
Legal status: Prescription medicine in most countries; not on the WADA Prohibited List.
What it gives
What people use it for and what the sources report. Strength is a general category from the literature: mild, clear or strong.
Purpose
A doctor may prescribe it for erectile problems linked to AAS, the hormonal low after stopping them, 19-nor steroids or finasteride, and for urinary symptoms when androgens enlarge the prostate. Medical use: erectile dysfunction, BPH symptoms, pulmonary hypertension.
What people seek and what studies show
Used for erections and for urinary symptoms of an enlarged prostate. In placebo-controlled trials in men with erectile dysfunction of many causes, including diabetes, it improved erections for up to 36 hours, and the effect did not fade over time. It works only with sexual arousal. Prostate symptom scores improved modestly. In small studies in healthy athletes it did not change aerobic or anaerobic performance.

Possible side effects
Possible does not mean certain: whether they appear depends on dose, duration, individual response and monitoring. Below — what to watch for and how to notice it early.

How to lower the risks
For 3 of 4 possible side effects there are specific ways to lower the chance: everyday measures, supplements and options a doctor may consider. For the rest — general advice. No measure removes a risk completely.
Stop and see an ophthalmologist at any vision change.
How each option helps (1)
- Working with a doctor — Vision changes on clomiphene, tamoxifen, isotretinoin, PDE5 or GLP-1 drugs need prompt review; several labels advise stopping, which may limit lasting harm.
Products obtained without a prescription are often fake, mislabeled or non-sterile, and there is no way to check at home. For legal supplements, look for third-party testing (Informed Sport, NSF).
Low-level side effects
How each option helps (2)
- Working with a doctor — Finds the cause of joint or muscle pain — estradiol pushed too low by an aromatase inhibitor, GH-driven swelling, statin muscle effects — so treatment can be adjusted.
- Coenzyme Q10 — May ease statin-related muscle aches (one meta-analysis positive, results mixed); no help for joint pain from low estrogen.
How each option helps (2)
- Working with a doctor — Assesses persistent vomiting or severe belly pain (possible pancreatitis or gallstones) and adjusts prescribed GLP-1 treatment when nausea persists.
- Pancreatin + dimeticone (Pankreoflat) — May ease heaviness and gas after meals; can cause nausea or diarrhea. Not for pork or cow's milk protein allergy (anaphylaxis reported) or early acute pancreatitis.

Myth & fact
“Cialis solves the libido and erection problems caused by nandrolone.”
It helps the mechanics of erection, but low libido from high prolactin, off-balance estrogen or low testosterone needs its cause found.
Combinations
Tadalafil lowers blood pressure a little, and so do these drugs; the effects add up. With dehydration, heat or alcohol this can mean dizziness or fainting, especially when standing up quickly.
FurosemideHydrochlorothiazideSpironolactoneTelmisartanNebivololLosartan / valsartan (angiotensin receptor blockers)ACE inhibitors (ramipril, perindopril, lisinopril)AmlodipineBisoprololCarvedilolMelanotan II and PT-141 can trigger erections on their own, and priapism is reported with melanotan; with tadalafil the effects add up. An erection lasting over four hours is an emergency that can cause permanent damage.
