Bremelanotide

PT-141, PT141, Vyleesi, Bremelanotide acetate

Forms: Approved as an autoinjector (Vyleesi, US); unapproved 'PT-141' products have unverified content and sterility.

PeptidesPrescription onlyEvidence: approved medicine

A melanocortin-receptor agonist related to Melanotan II that acts on desire pathways in the brain. Approved in the US for low sexual desire in premenopausal women.

What it gives

What people use it for and what the sources report. Strength is a general category from the literature: mild, clear or strong.

Purpose

Used to boost libido and arousal; men use it off-label for erections. In trials the effect on desire was modest. Not approved for men or to enhance sexual performance.

What people seek and what studies show

People hope for stronger desire and arousal; men also hope for easier erections. In two phase 3 trials in premenopausal women, desire scores rose modestly and distress about low desire eased, but the number of satisfying sexual events did not differ from placebo. It works through brain melanocortin receptors rather than blood flow; in early studies in men a nasal form produced erections more often than placebo.

Possible side effects

Possible does not mean certain: whether they appear depends on dose, duration, individual response and monitoring. Below — what to watch for and how to notice it early.

5 possible side effects

level · how often it is reported
Elevated
Black-market quality risk (fakes, contamination)
Most 'PT-141' outside the US pharmacy product is unregulated powder of unknown content and sterility.
How to notice: Unexpected effects, pain or infection, a stronger or weaker effect than expected.
possible
Low
Nausea, GI upset, pancreatitis risk
Nausea in about 4 in 10 users, usually easing with later uses; vomiting, flushing and headache also occur.
How to notice: Nausea, fullness, constipation or diarrhea.
common
Low
Higher blood pressure
Raises blood pressure slightly (up to about 6 mmHg) for up to 12 hours after each dose and slows the pulse; contraindicated in uncontrolled hypertension or heart disease.
How to notice: Often silent; sometimes headaches, flushing, nosebleeds — a home monitor shows it.
common
Low
Injection pain, infection, oil cough
Injection-site reactions occur in about 1 in 8 users.
How to notice: Soreness, lumps; a short coughing fit right after an injection.
common
Low
Skin darkening, changing moles
Dark patches on the face, gums or breasts: about 1% with occasional use but 38% with use every day in one study; more likely with darker skin, may not fade.
How to notice: Darker skin and freckles, new or darker moles.
possible

How to lower the risks

For 3 of 5 possible side effects there are specific ways to lower the chance: everyday measures, supplements and options a doctor may consider. For the rest — general advice. No measure removes a risk completely.

ElevatedBlack-market quality risk (fakes, contamination)

Products obtained without a prescription are often fake, mislabeled or non-sterile, and there is no way to check at home. For legal supplements, look for third-party testing (Informed Sport, NSF).

Low-level side effects

Digestion
Smaller meals, enough fluid and fiber; a doctor for severe abdominal pain.
Measure
Working with a doctor
Supplement or OTC product
How each option helps (2)
  • Working with a doctor — Assesses persistent vomiting or severe belly pain (possible pancreatitis or gallstones) and adjusts prescribed GLP-1 treatment when nausea persists.
  • Pancreatin + dimeticone (Pankreoflat) — May ease heaviness and gas after meals; can cause nausea or diarrhea. Not for pork or cow's milk protein allergy (anaphylaxis reported) or early acute pancreatitis.
Blood pressure
Less salt, regular cardio, good sleep; home readings; a doctor if it stays high.
Measure
Home blood-pressure monitoringRegular cardio trainingWorking with a doctorAvoiding alcohol and other liver stressors
How each option helps (8)
  • Home blood-pressure monitoring — Seated, rested readings on several days show your true BP; persistently high values are a signal to see a doctor.
  • Regular cardio training — Regular aerobic training lowers resting BP by a few mmHg, more in people with high BP.
  • Working with a doctor — Diagnoses persistent hypertension and prescribes treatment when needed.
  • Avoiding alcohol and other liver stressors — Regular drinking raises blood pressure; cutting it out lowers it.
  • Coenzyme Q10 — Inconsistent: some analyses show a small systolic drop in cardiometabolic disease, but a Cochrane review found no clinically meaningful effect.
  • Magnesium — Meta-analysis of trials: about 2 mmHg lower systolic pressure on average.
  • Omega-3 (fish oil, EPA/DHA) — Small drop of a few mmHg, mainly in people with untreated high blood pressure.
  • Taurine — Meta-analysis of 20 small trials: about 4 mmHg lower systolic pressure on average.
Injection site
Sterile technique, never share needles, vials or pens; see a doctor for a hot, swollen site. If equipment was ever shared, test for HIV and hepatitis B/C and ask about hepatitis B vaccination.
Moles
Sun protection, a mole map and regular dermatologist checks.
Measure
Dermatologist mole checks
How each option helps (1)
  • Dermatologist mole checks — Melanoma caught early is usually curable by removal; checks don't stop moles from darkening or multiplying on melanotan, they only catch danger early.

For women

Approved only for premenopausal women with distressing low desire; animal data suggest possible fetal harm, so it is not for use in pregnancy.

Myth & fact

Myth

“PT-141 is 'Viagra for the brain' and works for everyone.”

Fact

It acts on desire pathways, but in trials the effect was modest, nausea was common and blood pressure rose after each use.

Combinations

CautionTadalafil with melanotan II or PT-141

Melanotan II and PT-141 can trigger erections on their own, and priapism is reported with melanotan; with tadalafil the effects add up. An erection lasting over four hours is an emergency that can cause permanent damage.
