Long-chain omega-3 fats EPA and DHA from fish, krill or algal oil; prescription forms treat very high triglycerides. High intake slightly increases bleeding tendency, notably with blood thinners or aspirin.

Purpose

Taken for heart and joint health and to soften the effect of steroids on blood lipids. It reliably lowers triglycerides and slightly lowers blood pressure, but does not fix the HDL drop or LDL rise caused by steroids.

What people seek and what studies show

At prescription strength it lowers very high triglycerides by 30% or more. In the large VITAL trial in older adults at usual risk it did not cut major heart events. In high-risk statin users purified EPA cut events by a quarter (its placebo choice is debated); a high-dose EPA+DHA drug did not. Joint pain and stiffness in inflammatory arthritis ease modestly; small trials hint at less soreness after hard exercise.

Myth

“The more fish oil, the healthier the heart.”

Fact

Large trials found little benefit in healthy people, and atrial fibrillation risk rises with dose. Omega-3 lowers triglycerides; it does not undo the HDL crash from steroids.

Combinations

CautionAspirin with omega-3 or vitamin E

Aspirin makes platelets less sticky, and vitamin E and omega-3 oils add milder effects on clotting of their own. Gum bleeding was more common with vitamin E plus aspirin, and bleeding with a prescription omega-3 more common in people also taking aspirin. Black stools, blood in urine or unusual bruising are reasons to see a doctor.
