Creatine monohydrate
Creatine HCl, Creapure, Kre-Alkalyn
Forms: Monohydrate is the studied standard; HCl, buffered, ethyl ester and nitrate forms have shown no advantage over it.
A natural compound stored in muscle as phosphocreatine that rapidly recharges ATP during short, intense efforts. The most researched sports supplement, with a strong safety record in healthy people.
Legal status: Sold freely as a supplement and not prohibited by WADA; tested athletes should still choose third-party-certified products because of contamination risk.
What it gives
What people use it for and what the sources report. Strength is a general category from the literature: mild, clear or strong.
Purpose
Used to top up muscle creatine stores: more reps and power in hard sets, faster strength and lean-mass gains with training, better repeated sprints. Vegetarians, with lower baseline stores, often respond most.
What people seek and what studies show
Very well studied. With resistance training, meta-analyses show about 1 kg more lean mass than training alone (more in men than women; partly water) and small extra strength gains in efforts under 3 minutes; without training, lean mass was unchanged. Prolonged endurance barely improves. After stopping, muscle stores return to baseline over 4–6 weeks, not below it. Data on memory and sleep loss are early and sparse.

Possible side effects
Possible does not mean certain: whether they appear depends on dose, duration, individual response and monitoring. Below — what to watch for and how to notice it early.

How to lower the risks
For 2 of 3 possible side effects there are specific ways to lower the chance: everyday measures, supplements and options a doctor may consider. For the rest — general advice. No measure removes a risk completely.
Low-level side effects
How each option helps (1)
- Working with a doctor — Tells harmless water retention from swelling due to heart, kidney or liver trouble; steroid labels warn that edema can come with heart failure.
How each option helps (2)
- Working with a doctor — Assesses persistent vomiting or severe belly pain (possible pancreatitis or gallstones) and adjusts prescribed GLP-1 treatment when nausea persists.
- Pancreatin + dimeticone (Pankreoflat) — May ease heaviness and gas after meals; can cause nausea or diarrhea. Not for pork or cow's milk protein allergy (anaphylaxis reported) or early acute pancreatitis.
Check-ups and blood tests
Which tests and check-ups to do and when. This is a guide — agree the exact schedule with your doctor.
For women
Benefits women too, though some studies show smaller strength and muscle gains than in men; side effects are minimal, and early weight gain is water inside muscle, not fat.
Myth & fact
“Does creatine damage the kidneys or cause hair loss?”
Not in healthy people: studies of up to five years show no kidney harm, and a higher blood creatinine is expected without real damage (a cystatin C test clarifies). The hair-loss idea rests on one 2009 DHT study; a 2025 RCT found no effect on DHT or hair.
